Preparing for your treatment
Our goal is to guide you through the process and provide you with the necessary tools to enjoy the journey. Preparation requires an assessment by your provider of your readiness and having a sense of connection between you and your provider. We are engaging in a therapeutic endeavor to benefit you. Together, we are creating a state of mind in a safe and comfortable setting.

For ease of your visit, we request that you download and fill out this form before your visit. Please bring this form with you to your first appointment.
We ask that you please arrive 15 minutes early for your appointment time in order to get checked in and review your paperwork.
Policies & Procedures
Insurance
Hampton Insight does not accept insurance. We will give patients a copy of invoices to submit to insurance or healthcare spending accounts. Patients are responsible for submitting these bills and Hampton Insight will not be held accountable if insurance does not reimburse the full amount or pay in a timely manner.
Cancellation and Rescheduling
We ask you to please make every effort to not reschedule or cancel your session. If you are unable to attend your appointment, please give us a 48 hours notice for any 45-minute meeting and 1-week notice for the Ketamine Assisted Psychotherapy Medicine Sessions.
If this is not possible, due to the intensive nature of this therapy, you will be required to pay for half of the amount of the appointment. Any failure to comply with the treatment prerequisites will also result in having to pay half of the session’s cost.
